To complete your application, we must verify the age of all household members under the age of 18. A birth certificate is one acceptable option, but it is not required and is usually best used only if other records are not available. Below are examples of other reliable third-party documents that may be accepted. Each document must clearly show the child’s name and date of birth. Copies are sufficient; originals are not required.
